Services that are Information Sharing Entities (ISEs) or Risk Assessment Entities (RAEs) can request information from Dental Health Services Victoria under the Family Violence Information Sharing Scheme (FVISS) or Child Information Sharing Scheme (CISS).

If you are an ISE or RAE and wish to request information from DHSV under one or both schemes, complete this form and email it to infosharing@dhsv.org.au.
